To the State Board of Charities and Contributors of the Western Pennsylvania Hospital: Gentlemen:— It is a source of great pleasure to the Board of Di- rectors to note the continued upward trend of the Hospital, as evidenced by the technical medical report, herewith sub- mitted, and the growing popularity of the institution, as manifested by the increased number of patients and the general appreciation of the public. It was predicted that the change of location to the present delightful surroundings would so alter the char- acter of our clientele, as to practically subvert the wishes of the founders of this institution. Such, however, has not been the case, we are most happy to say. While it is true that a class of patients is now attracted to the Hos- pital that we could not have dreamed of securing at Twenty-Eighth Street, the directors have remained true to the traditions and the pledges made by the incorpora- tors, namely, “not to forget the needy”—for we continue to do fifty per cent of charity work without in any way marring the privacy and comfort of the well-to-do, yet, at the same time, affording the indigent a beautiful park outlook, with every possible comfort that hygienic, dietetic and scientific treatment can devise. Hospital management is frequently criticized for wasteful expenditure of money, and that, by business peo- ple, who, if they would but take the time to investigate. would, instead of being carpers, become fast friends of our public institutions. The Western Pennsylvania Hospital is THE WESTERN PENNSYLVANIA, HOSPITAL 17 not a corporation for profit. The directors and their of- ficers receive no salary, and it is therefore difficult to Se- cure able business men to join the directorate, as in addi- tion to the foregoing, a director is forbidden by law to furnish supplies to the hospital, through which he might derive profit. When one considers that in a dull summer month, or about Christmas time, that we average an employee per patient, not counting the medical staff and a host of medi- cal assistants, whose services are not a charge upon the Hospital, it is a little short of marvelous that the cost of maintenance per patient should only amount to from One Dollar and a Half to Two Dollars per day—less than the cost of an ordinary hotel room. The State now considers and treats hospitals as post graduate schools for young medical men and prescribes certain requirements and standards. There is in this in- stitution, as in all educational and progressive institutions of any kind, a constant need for improvement and the raising of standards, that cannot be done without pecuniary assistance from the State and philanthropic citizens of Pittsburgh. Water, that heretofore was free, is now, by civic en- actment, taxed to the amount of Six Thousand Dollars per year, in spite of its economic use by meter. The foundations of the dormitory still lie bare, await- ing a superstructure. A good dormitory is an essential feature to all hospitals, as attractive surroundings invite better educated and more refined young women, and they in turn give a greater equivalent than could be expected from those not so well endowed. A nurse must have talent and aptitude, but training, as in every other calling, is an essential requisite to a finished education. We sincerely hope that this great need of the hospital will be met this year by the State Legislature and the friends and sup- porters of the institution in this community. The direc- tors in the past have been more than willing to fulfill their duties in this direction and will not evade them in the fu- 18 THE WESTERN PENNSYLVANIA HOSPITAL ture, but feel that this is a burden that should be borne by broader shoulders. Beautiful bronze tablets have been placed through- out the hospital, to keep green the memory of those who have made bequests to the institution, while living con- tributors can see recorded upon our walls in enduring form, their names, in evidence of the fact that the direc- tors are ever mindful of the help and generosity of all who have or have had the interests of the Hospital at heart. The Board felicitates itself on the cordial relations existing between the officers of the various departments of the Hospital and on the fidelity that has characterized the work of the employees. The Medical and Surgical Staff, through their skill and devotion to duty, continue to merit the respect and confidence of the Board of Directors in the highest degree. We have gone forward as far, perhaps, as dur- ing any year in our history. Yet if we attempted to appor- tion credit, our report would consist not of a story of the work of the clinicians, but would rather be a recital of the extent to which we have been helped by other departments. The great extension to practical medicine of highly Scien- tific advanced laboratory work has revolutionized the for- mer subject. It is only recently that the West Penn Hos- pital has taken its place in the front rank, so far as this subject is concerned. No one has benefited more than the patients of the medical men. The improved equipment and increased efficiency of the Roentgenological department lead us every day to greater use of this agency. In this way we attain results in diagnosis, otherwise quite impossible. So we might go on paying our debt of acknowledge- ment; to the nursing staff, the business management of the hospital and to the social service workers. The fact is, however, that the conditions under which we now work are so satisfactory as to favor the development of that self- sufficiency which leads inevitably to dry rot. It is advis- able, therefore, that an annual review should be devoted to the consideration of our shortcomings rather than of our advances. Our principal deficiencies in the medical wards are delay in elementary, routine laboratory work, limited porch space, and the lack of a few simple appliances for hydro-therapy, electro-therapy and photo-therapy. Possibly the most serious deficiency of all is that autopsies are rare and exceptional occurrences rather than the rule in fatal cases. This is the more to be regretted in that the patho- logical laboratory is so well equipped to perform complete THE WESTERN PENNSYLVANIA HOSPITAL 33 and accurate examinations and to preserve in accessible and permanent preparation, all specimens which may be of value for future scientific study. The MacKenzie Polygraph purchased last year has been in frequent use, with interesting results. The devel- opment of its usefulness, however, will require the assign- ment of a staff physician to make routine tracings in every cardiac case. The necessity for giving each embryo as- sistant a thorough grounding in laboratory work has so far prevented such an arrangement. The medical staff will, doubtless, be able to effect it in the course of the com- ing year. The Record Department has reached that state of ef- ficiency in which it is possible, with a minimum amount of delay, to secure the medical history of past inmates of the Hospital. We take pleasure in tendering our thanks to Dr. H. R. Decker of the Assistant Staff and the resident physicians, as without the co-operation of the latter, ac- curate records would be impossible. Gentlemen:— The Surgical Department reports a year of expansion and attainment in the field of Surgery and Research. We continue to receive a large number of industrial accident cases, as well as an increasing number of pathological pa- tients. The year's statistics show sixty-five per cent of those admitted to be surgical cases.” Hospital statistics based upon ungrouped diseases, or dissimilar diseases, are unreliable. In certain complications of disease the rate of mortality is one hundred per cent, while in others it is a negligible quantity. A “Follow-up System” was inaugurated August 1st, 1914, which will give reliable data as to ultimate end re- sults in all cases. Certain sections of the department deserve special no- tice, on account of their close connection with the Surgi- cal Department. Efficient ambulance and admittance services are demanded by patients and public alike; to that end two resident physicians devote their time to this serv- ice, insuring immediate attention. The Department of Anaesthesia has been placed in charge of an expert, Dr. J. R. McCurdy, in accordance with the new law, in addition to the professional anaesthetist that we have had for years. The establishing of facilities for making and examin- ing frozen sections on the operating rooms' floor, when the question of a tumor's malignancy is to be decided, not only facilitates Pathologist Andrew’s work, but is a great fac- tor as to time, both of which are of the utmost moment to a patient undergoing an operation under anaesthesia. The surgical pathology emanating from the Labora- *Wiz, 4,236 surgical patients, who required 4,790 operations. 42 THE WESTERN PENNSYLVANIA HOSPITAL tory under Director Brenner is of a high order, as evi- denced by his report and the recent exhibit of the Hospital at the Carnegie Institute during the State Medical Socie- ties’ meeting here. The operating room section (comprising in all, six operating rooms with proper anaesthetizing facilities), re- quires some consideration. Efforts are being made to minimize the shock that is incidental to all operations, by keeping the operating room floor as quiet as possible, and freed from any display of the elaborate modern arma- mentarium essential to present surgical technique. We take great pleasure in commending the admirable work of Miss Turnbull and Miss Bower, in carrying out the details of the operating room technique, without which the surgeon’s work would be of no avail. The Surgical Department of the Staff cannot allow this opportunity to pass without giving its meed of praise to all medical assistants for the devotion that they have shown to duty during the past year. This report, therefore, covers the period which marks the transition from the old laboratory system to the present. It was divided into three sections; Bacteriological, Pathological and Biochemical, in charge of Dr. J. Bronfer-Brenner, Dr. V. L. Andrews and Dr. J. Rosenbloom respectively. There was also organized a routine section which was put in charge of the Interne Staff under the direct supervision of the Chiefs of Section. Dr. Brenner was appointed director of this department. The pavilion of the Pathological and Research De- partment was remodeled and its laboratories were equipped with all the modern appliances, thanks to the generosity of Messrs. J. C. Trees, M. L. Benedum and H. S. Grayson. Under these changed conditions great progress was made in all the sections of the department. Not only has the number of laboratory examinations increased, but the work itself was put on a more modern and scientific basis. New fields of laboratory work, such as metabolic studies and antirabic treatments, were opened and, wherever it was possible, new and improved methods of laboratory investi- gations were introduced. Our Laboratories were among the first to use Luetin test for syphilis and is the only one, outside of Nogouchi’s Laboratory, to prepare its Luetin. Dr. Bronfenbrenner in- troduced his new test for tuberculosis and utilized Besred- kas' Antigen in another test for the same disease. Wasser- man test became part of the routine laboratory work and Swifts-Ellis treatments quite a common practice. Improve- ments in recording and filing laboratory findings were in- troduced; a new system of collection of specimens and dis- tribution of reports was developed and facilities for re- search work were established. A number of problems are at present engaging the attention of our Laboratories and Medical Staff and a num- THE WESTERN PENNSYLVANIA HOSPITAL 81 ber of important contributions have already appeared in several publications. Our Laboratories made wonderful progress during the last year. The purchase of the new up-to-date equipment and the establishment of a nursing service in this department greatly facilitated and advanced the Radiographic and Fluoroscopic work of our Hospital. The department needs cold water pipe connection for THE WESTERN PENNSYLVANIA HOSPITAL 83 developing of plates. The water in this department is too warm, and Dr. Grier is compelled during the summer months to develop the Hospital plates at his office. His suggestion of connecting the water pipes with the drink- ing water system is worth considering. PHARMACEUTICAL DEPARTMENT Mr. A. A. Munsch, Apothecary. Prescriptions last year. . . . . . . . . . . . . . . . . . . . 2,745 Prescriptions the year before. . . . . . . . . . . . . 3,100 This reduction in number of prescriptions is due to the gradual improvement in the pavilion-drug supply en- abling the nurse herself to fill “single dose” prescriptions, Such an arrangement brings about a great saving in drugs and eliminates a great deal of waste. HYDROTHERAPEUTIC DEPARTMENT Dr. W. H. Cameron, Director. This department has not yet been organized. There is, however, a great need for it, especially for a continuous bath equipment. In the Electrotherapeutic Department the Hospital purchased during the year an electrical instrument pro- ducing all the necessary current for therapeutic use, two vibrating machines and an electrical leg baking machine. Five to eight patients are being treated daily with these ma- chines. BIGGER, Superintendent. 90 THE WESTERN PENNSYLVANIA HOSPITAL To the Chairman and Members, Training School Committee, The Western Pennsylvania Hospital. Gentlemen:— I have the honor to submit the report of the Training School for Nurses for the year ending May 31st, 1914. The School was organized in the year Eighteen Hun- dred and Ninety-two. Since its establishment, three hun- dred and thirty-three nurses have been graduated. The majority of this number are engaged in private duty, though many may be found in responsible positions in representative institutions. The Annual Commencement Exercises were held in the Conservatory of Music, on the evening of April 28th— twenty nurses received diplomas. General Albert J. Logan presided. Mr. James D. Hailman, chairman of the Staff and Training School Committee, presented the diplomas and Dr. James W. Macfarlane, President of the Medical Board, presented the pins to the class. Dr. Maitland Alex- ander delivered an inspiring address, that was most fe- licitous and highly appreciated by both nurses and audience. The Conservatory of Music was beautifully decorated for the occasion, and through the kindness of the Training School Committee, the officers of the school were recipients of beautiful flowers. The health of the nurses has been generally good throughout the year. The number of cases of tonsillitis has been less than in previous years. Three nurses Were operated upon for appendicitis and another developed Ty- phoid Fever. All made excellent recoveries. Early in the year, the Cot Club presented the Hospital with a projectoscope, which was placed in the Training School Department and is used to great advantage by the members of the faculty. The members of the Staff continue to give regular THE WESTERN PENNSYLVANIA HOSPITAL 91 courses of lectures and demonstrations in their various specialties. The efforts of the Organization were at first tenta- tive, but each succeeding year has added to the scope and usefulness of the Association. In the space allotted to us, we can only mention the most salient features of our work. In addition to main- taining a private room for some years back, we furnished the Children’s Ward in the present Hospital and maintain it entirely, besides paying for the services of the graduate head nurse in charge of the children. In June of this year, we presented to the Hospital the most scientific and practical projectoscope that could be obtained, with the view of facilitating the teaching faculty of the nurses. College for Women. 96 THE WESTERN PENNSYLVANIA HOSPITAL It is hardly necessary to explain the need of a Social Service Department in a large hospital or dispensary, as it is now a generally recognized fact that out of the hun- dreds of sick and suffering people who come to the hos- pital for relief, a very large number owe their troubles to bad housing, bad habits, ignorance, financial difficulties and other non-medical, non-surgical causes, which the busy doc- tor or nurse cannot investigate or remove. These patients require social diagnosis and social treatment. Illness can often be prevented by investigation and correction of the conditions under which these people live and work. Our department is charitable in a sense, yet we aim to keep al- ways before us the ideal of medico-social service, which should be, by a more intimate knowledge of the patient’s social background “to assist in making diagnosis more com- plete and medical treatment more effective.” The second year of the Social Service Department shows a marked increase in point of scope and effective- ness. Every free patient is now interviewed the morning following admission, regarding his or her home condition, and a short social history is kept on file in the office of the department. Through the Clearing House of Charitable In- formation we then learn what other agencies in the city are interested in the case, and with them we are in con- stant co-operation to meet as adequately as possible the needs of each patient. The Auxiliary Committee was recently organized for the purpose of supporting a graduate nurse, who visits the patients in their homes after discharge from the hospital, in order to see that the prescribed treatment is continued or properly carried out. This Committee, having an inti- mate knowledge of the work of the department, will also act in an advisory capacity in cases presenting special dif- ficulties. We wish to thank the members of the Volunteer Com- THE WESTERN PENNSYLVANIA, HOSPITAL 97 mittee for their assistance, and particularly Miss Edna Schoyer, who comes to the Hospital Monday afternoons to teach the ward patients basket making. Several of the doc- tors have expressed their appreciation of the value of this work for nervous patients, in giving them a pleasant in- terest and occupation. This work will be continued. Through the Volunteer Committee, plans are being made to teach and to have kindergarten games for children who will be prolonged hospital cases. The outlook for the coming year is very encouraging, and with the additional support of our Auxiliary Commit- tee, we hope to continually increase our helpfulness to the patients and our usefulness to the hospital.
