auf die therapeutischen Empfehlungen der Standardmedikamente auch bei HFmrEF ("heart failure with mildly reduced ejection fractiion"), die auch bei HFrEF ("heart failure with reduced ejection fractiion") eingesetzt werden, d. h. die Vierfachkombination aus Angiotensin-Rezeptor-Neprilysin-Inhibitor (ARNI) bzw. ACE("angiotensin-converting enzyme")-Hemmer, Betablocker und Mineralokortikoidrezeptorantagonist (MRA). Eine echte Neuentdeckung ist die Klasse-I-Empfehlung für die Behandlung mit SGLT2("sodium-glucose linked transporter 2")-Inhibitoren (SGLT2i) bei Patienten mit HFrEF unabhängig vom Vorliegen eines Diabetes mellitus. Da spielt die neue Benennung der Herzinsuffizienz mit mild reduzierter Ejektionsfraktion (HFm-rEF; EF: 41-49 %, früher "mid-range", jetzt "mildly reduced") nur eine Nebenrolle. Als Ergänzung des medikamentösen Therapiespektrums bei einer Verschlechterung der Herzinsuffizienz bei Anwendung der "fantastic four" [5] hat Vericiguat, ein Stimulator der löslichen Guanylatzyklase, eine IIB-Empfehlung erhalten [6] .
