I, Jena, Deutschland Hintergrund: Seit den Publikationen von Holzer et al und Bernard et al aus dem Jahr 2002 wird auf unserer Intensivstation die therapeutische Hypothermie bei Pat., welche nach Reanimation komatös bleiben, angewendet. Dabei beschränken wir uns im klinischen Alltag nicht nur auf die von der ILLCOR empfohlene Indikation, sondern setzten die Hypothermie großzügig auch bei Pat. mit Asystolie und nach Reanimation im Krankenhaus ein. Mit den folgenden Daten sollen unsere Ergebnisse und Erfahrungen mit der therapeutischen Hypothermie dargestellt werden. Methode: In einer retrospektiven Analyse untersuchten wir 81 Patienten (17 Frauen, 64 Männer, mittleres Alter 59,3 Jahre), die wegen anhaltender Bewusstlosigkeit nach CPR über 24 h mit therapeutischer Hypothermie behandelt wurden und verglichen diese mit den Daten von 41 Patienten (12 Frauen, 29 Männer, mittleres Alter 63,8 Jahre), die eine Standardtherapie ohne Hypothermie erhielten. Alle Patienten mussten mindestens 48 h überleben. Bei Pat. die eine Hypothermiebehandlung erhielten wurde vor Kühlbeginn ein Aufwachversuch von 45 bis 60 Min. Dauer durchgeführt. Um eine Negativauswahl bei den Patienten mit Standardtherapie ausschließen zu können, wurden nur Patienten in die Auswertung eingeschlossen, welche die gleichen (erweiterten) Einschlusskriterien wie die Hypothermiepatienten erfüllten. Ergebnisse: Von 81 Hypothermiepatienten verstarben 33 (41%), 48 überlebten (59%). In der Standardtherapiegruppe verstarben lediglich 10 Pat. (24%, p = 0,074), 31 überlebten (76%). In dieser Gruppe überlebten signifikant mehr Pat. mit einem GOS von 3-5 als in der Hypothermiegruppe (68% vs. 48%, p = 0,035). In der Hypothermiegruppe wurden signifikant mehr Pat. prähospital reanimiert, 65% vs. 39% in der Standardtherapiegruppe. Bei Pat. mit Kammerflimmern verstarben in der Hypothermiegruppe 16 (31%), in der Standardtherapiegruppe 3 (12%, p = 0,062) und bei Pat. mit Asystolie verstarben 17 (59%) vs. 6 (40%, p = 0,24). Pat. die in der Hypothermiegruppe mit einem GOS von 3-5 überlebten (n = 39) waren signifikant jünger (53,5 vs. 64,7 Jahre, p = 0,006) als Pat. die verstarben oder apallisch (GOS 1 + 2) wurden. Außerdem war in dieser Gruppe Kammerflimmern mit 77% vs. 52% signifikant häufiger (p = 0,021) und der SAPS II lag mit im Mittel 52,2 signifikant niedriger als bei Pat. mit GOS 1 + 2 mit 63,4 (p = 0,006). Bei Pat. mit einem GOS von 3-5 lag die Körpertemperatur während der Behandlung im Mittel 21 h im Zielbereich von 32,5-33,5 8C während Pat. mit GOS 1 + 2 im Mittel nur 12,6 h im Zielbereich lagen. Dieser Unterschied war signifikant (p < 0,001). Andere Parameter wie Hypoxiezeit (p = 0,053), Anteil prähospitaler Reanimation (p = 0,49), Latenz zwischen ROSC und Kühlbeginn (p = 0,99), Latenz zwischen ROSC und Erreichen der Zieltemperatur (p = 0,81), Gesamtdauer der Kühlung (p = 0,65) und Kühlmethode (extern vs. intravasal, p = 1) zeigten keine signifikanten Unterschiede. Diskussion und Schlussfolgerung: Unter der Voraussetzung gleicher Einschlusskriterien für beide Therapiegruppen konnten wir keinen Vorteil für die therapeutische Hypothermie nach Reanimation im Vergleich zur Standardtherapie sehen. Hauptgrund dafür ist die außergewöhnlich geringe Sterberate in der Standardtherapiegruppe. Verglichen mit den Letalitätsraten von Holzer et al (41%) und Bernard et al (51%) nach Hypothermie sind unsere Ergebnisse jedoch akzeptabel. Die konsequente Einhaltung der Zieltemperatur von 33 8C über einen möglichst langen Zeitraum scheint von entscheidender Bedeutung für das neurologische Ergebnis zu sein. Thermoregulation and outcome in patients after cardiac arrest Uray T., Sterz F., Holzer M., Laggner A.N., Behringer W. Department of Emergency Medicine, Medical University of Vienna, Vienna, Austria Purpose: Mild hypothermia showed to improve neurologic outcome in patients after cardiac arrest. We observed that after initial cooling some patients remained hypothermic for 24 hrs without further active cooling, which might reflect impairment of thermoregulation by ischemic brain damage. The aim was to evaluate, if the need for further active cooling during the first 24 hrs after cardiac arrest reflects neurologic outcome. Methods: Included in the study were patients resuscitated from cardiac arrest, and cooled with a novel cooling pad (EMCOOLSpad®) to oesophageal temperature (Tes) 32-34 8C for 24 hrs. Sedation and muscle paralysis was provided as standard of care. After initial cooling to 33 8C, Tes of 32-34 8C was controlled for 24 hrs by manually repeatedly removing or applying two new small parts of EMCOOLSpad® on chest and abdomen. The number of reapplications during 24 hrs was recorded, and correlated to neurologic outcome, which was evaluated in terms of cerebral performance categories (CPC 1 and 2, favourable outcome; unfavourable outcome) . Data are presented as median and interquartile range (25-75%). Results: Between 09/05 and 06/07, 45 patients were included. Favorable neurologic outcome was achieved in 5 (56%) of the patients who remained hypothermic without the need for active cooling (n = 9), as compared to 13 (36%) of the patients who needed active cooling (n = 36) (p = 0.4). There was no difference in the number of reapplications of cooling pads to maintain hypothermia between patients with favorable outcome vs. unfavorable outcome: 3 (0-5)/24 h vs. 3 (1-7)/24 h, p = 0.5, and there was no correlation between CPC and number of reapplications (rs = 0.002, p = 1.0). Conclusions: The number of cooling pads needed to maintain mild hypothermia for 24 hrs in patients after cardiac arrest does not reflect neurologic outcome. The reasons for individual differences in thermoregulation remain unclear and need further investigations. Patienten erfolgten Herzalarme seltener wegen eines Kreislaufstillstandes verglichen mit medizinischen Patienten (25, 1 vs. 42, 2%, p < 0, 0001) , jedoch häufiger wegen respiratorischer Insuffizienz (21,3 vs. 15,0%, p = 0,016). 40% aller klinikweiten CPR-Einsätze konzentrierten sich auf allein 4 Stationen -alle 4 an der Inneren Medizin. Die Überlebensraten nach CPR unterschieden sich hochgradig zwischen Patienten fünf internistischer Subdisziplinen (6 bis 40%, p < 0,001), nicht aber zwischen chirurgischen und nichtchirurgischen Fächern (25 vs. 21%, NS). Nachts erfolgten weniger Einsätze als in den anderen zwei Tagesdritteln (23,4% von 00 bis 08 Uhr, p < 0,0001), vor allem weniger Einsätze ohne CPR. Nach innerhospitaler CPR überlebten 22,3% bis zur Krankhausentlassung. Zwischen 2002 und 2006 stieg die Überlebensrate bis zur Krankenhausentlassung nach HAT-Einsätzen mit Reanimation (p = 0,037, lineare Regression), noch deutlicher bei Einsätzen mit Defibrillation (p = 0,007). Ersthelfer führten immer häufiger Defibrillationen durch (p = 0,004). Bei Einsätzen mit Reanimation durch Ersthelfer stieg die Überlebensrate zwischen 2002 und 2006 (p = 0,014) . Schlussfolgerungen: Engagement für und Investition in innerhospitale Notfallmedizin steigern die Überlebenschancen nach einem Kreislaufstillstand. Ersthelfer können dazu wesentlich beitragen. Die Überlebenswahrscheinlichkeit unterscheidet sich auffällig nach der Tageszeit des Kreislaufstillstands (besonders ungünstig zwischen 23 und 05 Uhr) und nach der internistischen Grunderkrankung (höchste Überlebensrate bei Patienten der Kardiologie). Medizinische Klinik, Klinikum Rechts der Isar, München, Deutschland Hintergrund: Der Extravaskuläre Lungenwasser Index (ELWI) war in mehreren Studien ein signifikanter Prädiktor der Mortalität. Ferner korrelierte der ELWI bei Patienten mit Sepsis und ARDS mit der PaO 2 /FiO 2 -Ratio und der pulmonalen Compliance. Allerdings stellt sich bei der steigenden Anzahl von übergewichtigen Patienten die Frage, welches Körpergewicht für die Indizierung des EVLW gewählt werden soll. Daher war es das Ziel unserer Studie, die Korrelation des EVLWI mit der PaO 2 /FiO 2 -Ratio und dem Oxygenations-Index (OI: mean airway pressure * FiO 2 * 100/ PaO 2 ) in Abhängigkeit verschiedener Körpergewichts-Indices zu untersuchen. Methoden: Bei 25 Patienten einer internistischen Intensivstation mit einem Body Mass Index > 25 kg/m 2 , wurden 370 Messungen des EVLW mittels PiCCO System (Pulsion, München) durchgeführt. Die EVLW-Werte wurden in Bezug gesetzt zum aktuellen Körpergewicht (ABW), "predicted body weight" (PBW), "ideal body weight" (IBW) und zum "adjusted body weight" (AdBW). Die jeweiligen Werte wurden mit der PaO 2 /FiO 2 -Ratio und mit dem Oxygenations-Index korreliert. Statistik: Spearman-Korrelation, SPSS-Software. Ergebnisse: n = 25; 16 Männer, 9 Frauen; Alter 63,4 ± 24,6 Jahre, APACHE-II 24,6 ± 9,2. Alle Patienten hatten eine Sepsis oder ein SIRS. Das nicht indizierte EVLW (Mittelwert ± SD) betrug 637 ± 286 ml. Die indizierten ELWI-Werte betrugen 7,2 ± 3,0 ml/kg (indiziert nach dem ABW), 10,1 ± 4,2 ml/kg (PBW), 10,4 ± 4,3 ml/kg (IBW) und 8,8 ± 3,6 ml/kg (AdBW). Der ELWI korrelierte mit den funktionellen Parametern PaO 2 /FiO 2 -Ratio und OI unabhängig von der Indexierung mit hoher Signifikanz (p < 0,001). Die Korrelationen mit der PaO 2 /FiO 2 -Ratio betrugen -0,438 (nicht indiziertes EVLW), -0,467 (ABW), -0,506 (PBW), -0,496 (IBW) und -0,493 (AdBW). Die Korrelationen mit dem Oxygenation Index lagen bei 0,446 (nicht indiziertes EVLW), 0,508 (ABW), 0,600 (PBW), 0,590 (IBW) und 0,562 (AdBW). Der Normbereich des ELWI liegt bei 3,0-7,0 ml/kg, Werte zwischen 7,1 und 10,0 ml/kg gelten als leicht erhöht, und Werte > 10,0 ml/ kg sind deutlich erhöht. Gemäß dieser Einteilung unterschied sich die prozentuale Verteilung der ELWI-Werte je nach Index erheblich: 61% : 30% : 9% für das ABW, 22% : 36% : 42% für das PBW, 19% : 30% : 51% für das IBW und 38% : 37% : 25% für das AdBW. Schlussfolgerungen: 1.) Bei übergewichtigen Patienten korrelieren EVLW und ELWI (indiziert für ABW, PBW, IBW und AdPW) signifikant (p < 0,001) mit der PaO 2 /FiO 2 -Ratio und dem OI. 2.) Die höchsten Korrelationskoeffizienten wurden für die Indizierung gemäß PBW gefunden. 3.) Obwohl die Korrelationskoeffizienten sich nicht sehr stark unterschieden (-0,467 bis -0,506 bzw. 0,508 bis 0,600), ändert sich die Zuteilung zu den klinisch relevanten Kategorien "normal", "leicht erhöht" und "deutlich erhöht" in Abhängigkeit vom verwendeten Index erheblich. 4.) Im Hinblick auf die jeweils größten Korrelationskoeffizienten bezüglich der funktionellen Parameter scheint die Indizierung bezüglich des PBW am geeignetsten. Einfluss von rhDNase auf die Dauer der künstlichen Beatmung bei Intensivpatienten Riessen R. 1 In einer doppelblinden, placebokontrollierten, randomisierten nationalen Multizenterstudie (LUFIT) wurden Patienten in zwei Strata, chirurgisch oder nicht-chirurgisch, eingeteilt. Die Therapie wurde innerhalb von 48 Stunden nach Beginn der künstlichen Beatmung begonnen und bis zur erfolgreichen Entwöhnung von der Beatmung, längstens jedoch für 21 Tage fortgeführt. Es wurden entweder 2,5 ml rhDNase oder NaCl 0,9% alle 12 Stunden endotracheal instilliert. Ergebnisse: Insgesamt wurden 162 nicht-chirurgische und 123 chirurgische Patienten randomisiert. In der rhDNase-Gruppe verstarben 15 Patienten (4 chirurgische), in der Placebogruppe 16 Patienten (5 chirurgische) . Bei den nicht-chirurgischen Patienten war die mediane Beatmungsdauer bei den rhDNase-Patienten um 5,2 Tage (7,5 Tage, CI 6-9,3 Tage) signifikant kürzer als unter Placebo (12, 6 Tage, CI 7, [9] [10] [11] [12] [13] [14] [15] [16] 9 Tage, p = 0, 016) . Bei den chirurgischen Patienten betrug die mediane Beatmungszeit 16,6 Tage (CI 11, (5) (6) (7) (8) (9) (10) (11) (12) (13) (14) (15) (16) (17) (18) (19) (20) (21) in der rhDNase-Gruppe und 11,7 Tage (CI 8, (4) (5) (6) (7) (8) (9) (10) (11) (12) (13) (14) (15) 6 Tage, p = 0,38) in der Placebogruppe. Diskussion: RhDNase reduziert die Beatmungszeit bei erwachsenen, nicht-chirurgischen Patienten insbesondere mit Pneumonie. Die prophylaktische Gabe von rhDNase zeigt bei chirurgischen Patienten keinen Effekt hinsichtlich der Beatmungsdauer. [1, 2] . So wird z. B. die Beendigung einer künstlichen Beatmung (wie im Fall P. Welby) fälschlicherweise als ,aktive/direkte Sterbehilfe' bezeichnet, die verboten sei [3] . Zielsetzung: Es bedarf einer neuen Terminologie für Entscheidungen am Lebensende, die den Betroffenen eine bessere Orientierungshilfe bietet. Ergebnisse: (1.) Medizinische und pflegerische Sorge am Lebensende ist eine grundlegende Pflicht. Hierzu zählt auch die palliative Versorgung des Patienten. Sollte eine lege artis durchgeführte Anästhesie oder Sedierung als Nebenfolge das Leben verkürzen, so ist dies gerechtfertigt. (2.) Nichtaufnahme, Reduktion oder Abbruch einer Behandlung haben sich an zwei normativen Punkten zu orientieren: (a) medizinischer Indikation; (b) Patientenwille. Ohne medizinische Indikation ist eine Behandlung nicht aufzunehmen bzw. zu reduzieren oder abzubrechen [4] . Das Konzept einer provisional intensive care [5] sollte für die Indikationenstellung herangezogen werden. Wünscht der entscheidungsfähige Patient einen Behandlungsverzicht (z. B. Welby [6]), so ist dies zu respektieren und stellt keine verbotene ,aktive/direkte Sterbehilfe' dar. Dennoch ist Reduktion oder Verzicht auf intensivmedizinische Maßnahmen nicht als ,Im-Stich-Lassen' aufzufassen. (3.) ,Beihilfe zum Suizid' und (4.) Sepsis is a common complication in patients with haematological diseases undergoing chemotherapy. Biochemical pathways involved in the development of sepsis are only partly understood, especially in this group of patients. In this study we analysed markers of inflammation/immune activation including tryptophan catabolism and mannose binding lectin (MBL) levels towards their prognostic significance in septic patients suffering from haematological malignancies and "non-haematological" diseases. Concentrations of C-reactive protein (CRP), interleukin-6 (IL-6), MBL, neopterin, tryptophan and kynurenine were prospectively measured in sera of 36 septic patients with haematological malignancies and in 8 patients with sepsis due to another underlying disease for at least 3 times (at baseline and at the onset of sepsis, as defined by high grade fever, clinical signs of systemic infection or haemodynamic instability (day 1) and at day 3, day 7). Concentrations of CRP, IL-6 and neopterin increased significantly from baseline levels to day 1 and 3 of sepsis and decreased on day 7, while tryptophan levels decreased significantly after the beginning of sepsis. Septic patients without haematological diseases showed a significantly higher tryptophan degradation rate than patients with sepsis of other causes, and increased concentrations of neopterin and CRP, while MBL levels were significantly lower in this group as compared to subjects suffering from haematological malignancies. Concentrations of inflammation markers were correlated with each other, enhanced tryptophan degradation coincided with immune activation cascades. Patients who died during follow-up (one month, n = 4) tended to have lower baseline MBL concentrations than survivors. Septic patients with haematological disease showed an impaired immune response compared to patients with sepsis of other cause. Angiopoietin-2 correlates with severity of illness and mortality in medical patients with severe sepsis Kümpers Objective: Endothelial activation is a devastating key event in sepsis that leads to impaired vasomotor tone, leukocyte recruitment and capillary leakage. Recently, the endothelial specific angiopoietin (Ang)-Tie ligand-receptor system has been identified as an essential regulator of endothelial activation in preclinical models of acute lung injury and endotoxemic shock. Binding of circulating angiopoietin-1 (Ang-1) to the Tie2 receptor protects the vasculature from inflammation and leakage, whereas binding of Ang-2 antagonizes Tie2 signaling and disrupts endothelial barrier function. Here, we ask whether levels of circulating Ang-1 and Ang-2 correlate with severity of illness, cardio-pulmonary function and mortality in critically ill medical patients with severe sepsis. Design: Prospective study. Setting: Medical ICU at a tertiary care university hospital. Patients: Circulating Ang-1 and Ang-2 were measured by immunoradiometric assay (IRMA) and enzymelinked immunoassay (ELISA) in sera of 29 healthy controls and 29 patients with severe sepsis. Measurements and main results: Median serum Ang-2 levels were significantly elevated in critically ill patients compared to healthy controls. In contrast, Ang-1 levels were significantly lower in patients compared to controls. Ang-2 levels correlated closely with SOFA and APACHE II scores, thus indicating severity of illness. No association was detected between Ang-2 and cardio-circulatory or pulmonary function as assessed by transpulmonary thermodilution technique. Ang-2 was identified being a strong predictor for survival that yielded the same discriminatory ability than the APACHE II score. Conclusion: A marked imbalance of the angiopoietin-Tie system in favor for Ang-2 potentially contributes to vascular barrier dysfunction in septic medical patients. Elevated Ang-2 levels correlated with disease severity and predicted outcome in medical patients with severe sepsis. Measurement of circulating Ang-2 as a readily available powerful biomarker may open new perspectives for riskadapted treatment in the ICU. Diagnostik durchgeführt. Bei allen Pat wurden proANP, proAVP, C-reaktives Protein (CRP) und CRB-65 Score bestimmt. Die Pat wurden über 28 Tage bezüglich des Überlebens nachverfolgt. Ergebnisse: Pro-ANP-und proAVP-Werte stiegen mit dem Schweregrad der CAP an gemessen mittels CRB-65 Score (p < 0,0001). Bei den 24 Pat, die im Follow-up starben, waren die proANP- (237, 0 [IQ 124, 0] vs. 93, 5 [IQ 47, 5] pmol/L, p < 0,001) und proAVP-Werte (44, 2 [IQ 32, 4] vs. 12, 4 [IQ 4, (9) (10) (11) (12) (13) (14) (15) (16) (17) (18) (19) (20) (21) (22) 5] pmol/L, p < 0,0001) bei Aufnahme signifikant höher verglichen mit den Werten der Überlebenden. In der ROC-Analyse bezüglich des Überlebens war die AUC für proAVP (0,86, 95% CI 0,83-0,89) und proANP (0,76, 95% CI 0,72-0,80) signifikant höher verglichen mit dem CRP und vergleichbar mit der AUC des CRB-65 (0,73, 95% CI 0,69-0,77). Pat, die beatmet werden mussten, hatten signifikant höhere proAVP-und PCT-Werte, während proANP und CRP nicht unterschiedlich waren bei beatmeten und nicht beatmeten Pat. Pat mit Schock zeigten signifikant höhere Werte für proANP, proAVP und PCT verglichen mit Pat ohne Schock. CRP-Werte waren nicht unterschiedlich bei Pat mit oder ohne Schock. Schlussfolgerungen: proANP und CT-proAVP sind viel versprechende neue Biomarker für die Risikostratifikation von CAP-Patienten. Sie zeigen Vorteile gegenüber rein inflammatorischen Markern und sind erhöht bei Pat, die aufgrund der CAP einen Schock aufweisen, beatmet werden müssen oder versterben. Medizinische Universität Innsbruck, Universitätsklinik für Innere Medizin, Innsbruck, Österreich Rezente Studien konnten einen günstigen Effekt von Statinen in der Therapie der bakteriellen gram-negativen Sepsis zeigen. Für eine optimale Endotoxin-Wirkung in vivo ist die Anwesenheit des Akutphasenproteins Lipopolysaccharid-binding-protein (LBP) erforderlich. Dieses wird hauptsächlich in der Leber auf entzündliche Reize gebildet. Wir haben untersucht, ob Atorvastatin (ATV) die hepatozelluläre LBP-Expression beeinflussen kann. HepG2-Zellen wurden mit Zytokinen stimuliert und der Effekt von ATV auf die LBP-Produktion mittels ELISA untersucht. Die biologische Aktivität des in den HepG2-Überständen enthaltenen LBP konnte in Monozyten anhand der ICAM-Expression in FACS-Analysen gezeigt werden. TNF alpha und Interleukin-6 konnten beide die LBP-Sekretion stimulieren, wobei durch Zugabe von Dexamethason eine weitere Steigerung dieses Effektes zu erzielen war. Ohne Dexamethason konnte die LBP-Produktion in den HepG2 inhibiert werden, wohingegen diese Wirkung durch das Steroid aufgehoben wurde. ATV wirkte dosis-und zeitabhängig inhibierend, wobei bereits durch 1 Mikromol ATV eine signifikante Hemmung erzielt wurde. Überstände von HepG2, die durch Inkubation mit ATV deutlich weniger LBP enthielten, haben schlussendlich auch die LPS-Wirkung in Monozyten abschwächen können. ATV kann die LBP-Produktion in humanen Hepatozyten vermindern. Diese immunomodulatorische Wirkung könnte zu den positiven Effekten von Statinen bei Patienten mit gram-negativer Sepsis beitragen. Allgemeine Innere Medizin, Innsbruck, Österreich Der Zusammenhang zwischen der Leberfunktion und der Elimination bzw. der Pharmakokinetik von Amphotericin B (AMB) wurde bisher nicht untersucht. In dieser Phase IV-Studie wurde die Pharmakokinetik von kolloidalem AMB (ABCD) bei kritisch-kranken Patienten mit cholestatischem Leberversagen untersucht. Es wurden Zeit-Konzentrations-Profile bei kritisch Kranken mit cholestatischem Leberversagen und bei kritisch Kranken mit normaler Leberfunktion, die aufgrund einer invasiven Pilzinfektion mit ABCD behandelt wurden, ermittelt. Mittels Festphasenextraktion wurden der lipidformulierte und der freigesetzte Anteil von AMB getrennt und anschließend chromatographisch via HPLC quantitativ bestimmt. Drei Patienten mit eingeschränkter und drei Patienten mit normaler Leberfunktion am Tag 1 der antimykotischen Therapie wurden bislang eingeschlossen. Nach einer ABCD Einzeldosis (2,46 ± 0,54 Prometheus® ist ein extrakorporales Leberunterstützungsverfahren, welches auf der Methode von Fractionated Plasma Separation and Adsorption (FPSA) beruht und sowohl proteingebundene wie wasserlösliche Substanzen effektiv entfernt. Ziele: Untersuchung der Ef-fizienz des Prometheus®-Systems in Bezug auf die Entfernung von hohen ADMA-Spiegeln bei Patienten mit Leberversagen. Methodik: Das Patientenkollektiv bestand aus 8 Patienten mit akut-chronischem Leberversagen und gleichzeitigem Nierenversagen. Der mittlere MELD-Score lag bei 22 ± 7 und 7/8 Patienten lagen auf der Intensivstation. Alle Patienten wurden an zwei konsekutiven Tagen mit der Prometheus®-Therapie für je 5±1 Stunden behandelt. Die Plasma-Konzentrationen von ADMA und seinem Stereoisomer SDMA wurden mittels Massenspektrometrie und Gaschromatographie bestimmt. Ergebnis: Die Höhe der ADMA-Konzentrationen war mit dem MELD-Score korreliert (r s = 0,58, p < 0,0001). Vor dem Start der Prometheus®-Therapie waren die ADMA-und SDMA-Spiegel erhöht (1,3 ± 0,5 lmol/l bzw. 1,7 ± 0,4 lmol/l). Unter Prometheus® fielen die Plasmaspiegel von ADMA im Mittel um 25% (p < 0,0001) und die von SDMA um 22% (p < 0,0001). Allerdings trat ein signifikanter Rebound der ADMA-Konzentrationen zwischen den beiden Therapiesitzungen auf (p < 0,01). Schlussfolgerung: Diese Studie konnte zum ersten Mal zeigen, dass ADMA-Plasmaspiegel effektiv durch ein Leberunterstützungsverfahren gesenkt werden können. Dies könnte eine pathophysiologische Rolle bei den klinischen Effekten der Systeme spielen, da für ADMA eine Senkung von zerebralem und renalem Blutfluss sowie des Herzzeitvolumens gezeigt wurde. Purpose: OOH surface cooling for induction of mild hypothermia in patients after cardiac arrest was shown to be feasible with a novel cooling pad (EMCOOLSpad®), independent of any energy source during use. The aim was to evaluate feasibility of an improved design in the OOH setting. Methods: Included in the study were patients successfully resuscitated from OOH cardiac arrest with oesophageal temperature (Tes) > 34 8C. The cooling pad consists of multiple cooling units, filled with a mixture of graphite and water, stored in a cooling box at -3 8C. Cooling was initiated as soon as feasible until Tes 34 8C, when the cooling pads were removed. In phase I, pads with different sizes for predefined body parts with total surface area of 0.66 m 2 were used; in phase II, pads with uniform size, without predefined body assignment, covering 0.44 m 2 , were used. Data are presented as median and interquartile range (25-75%). During initial cooling, the blanket had to be exchanged due to depleted cooling capacity in 3/15 patients in phase I, and in 7/8 patients in phase II (p = 0.006). Conclusions: EMCOOLSpad® showed to be safe and feasible for rapid induction of mild hypothermia in the OOH setting. Cooling was started as soon as feasible after admission until Tes 34 8C was reached, when the cooling pads were removed. The target temperature of Tes 33 8C was kept for 24 hours. We tested two different filling masses of EMCOOLSpad®, in phase I 5.3 kg, and in phase II 6.2 kg. Data are presented as median and interquartile range (25-75%). Results: Between 05/06 and 05/07, 7 patients were cooled in phase I, and 10 patients in phase II. Baseline Tes was 36.4 (35.8-37.1) 8C in phase I, and 35.8 (35.3-36.0) 8C in phase II (p = 0.2). EMCOOLSpad® decreased Tes from baseline to 33.0 8C with a cooling rate of 2.8 (1.6-3.7) 8C/h in phase I vs. 2.9 (1.8-3.9) 8C/h in phase II (p = 0.7). To reach 34 8C, the cooling-pads had to be exchanged with a new one due to depleted cooling capacity in 5/7 patients in phase I, and in no patient in phase II (p = 0.003), reflecting improved practicability in phase II. In none of these patients minor or major device related adverse reactions were observed. Conclusions: The filling volume of EMCOOLSpad® did not influence cooling rate, but cooling capacity, in the initial induction of mild hypothermia in patients after cardiac arrest. In klinischen Studien nach Herz-Kreislauf-Stillstand erwies sich die Gabe großer Mengen kalter Kochsalzlösung (KKS) zur Induktion von Hypothermie als sicher und effektiv. Deshalb untersuchten wir in einer Pilotstudie die KKS-Effekte bei Patienten mit akutem ischämischen Schlaganfall. Methoden: 10 Schlaganfallpatienten mit Symptombeginn vor weniger als 3 Stunden wurden in die Studie eingeschlossen. Nach dem initialen CT wurden sie mit KKS (4 8C, 25 ml/kg Körpergewicht) und rt-PA behandelt, wenn die Kriterien für eine Thrombolysebehandlung erfüllt waren. Zur Prophylaxe und Therapie des Kältezitterns wurden Buspiron und Pethidin verabreicht. Nach Erreichen des Zielvolumens der KKS wurden keine weiteren Maßnahmen zur Kühlung getroffen. Während der Hypothermiephase wurden Zitterscores erhoben. Ergebnisse: Die Patienten hatten bei Aufnahme einen medianen National Institutes of Health Stroke Scale (NIHSS) Score von 5,5 (Range 4 to 12). 9 Patienten erhielten rt-PA innerhalb des 3-Stunden-Zeitfensters (104 ± 25 min). Die KKS wurden 17 ± 11 min nach Beginn der rt-PA-Gabe infundiert. Die tympanale Temperatur fiel um maximal 1,6 ± 0,3 8C (p < 0,005) 52 ± 16 min nach Beginn der KKS-Gabe. Die Patienten erhielten ein mittleres Volumen von 2163 ± 256 ml KKS. Bis auf geringes Kältezittern bei 2 Patienten zu Beginn der Infusionen kam es zu keinen Nebenwirkungen. Der mediane NIHSS Score verbesserte sich signifikant auf ein Median von 3 (Range 1 bis 15) nach 24 Stunden im Vergleich zur Aufnahme (p < 0,02). Introduction: Each year 17 million people worldwide die from cardiovascular disease. Persons who seek medical care for symptoms of myocardial infarction (MI) must decide on a mode of transportation to hospital. Interestingly, one of the primary reasons cited by persons with chest pain for not calling emergency medical services (EMS) for transport to the hospital is that private transportation is thought to be quicker. The aim of the study was to examine differences in mortality for patients with MI who used private transportation compared to patients who used EMS to reach definitive medical care. Methods: We retrospectively evaluated patients with primary admission of acute myocardial infarction who were admitted to the emergency department (ED) of a tertiary university hospital from January 1995 to December 2005. The primary endpoint (mortality at 30 days) was evaluated via the Austrian general mortality database (Statistic Austria). Follow up was done from day of admission until death or 31. 12. 2005. We used a Poisson model to assess the effect of self-admission on mortality rate while adjusting for variables that were potential confounders in the univariate analysis. Results: Of the study population of 3562 patients 2638 (74%) were admitted via EMS, 2662 (75%) were male, the median age was 59 years. Patients who arrived at the ED via private transportation had a significant better outcome in comparison to those patients who were admitted via EMS (OR 0.37, 95% CI 0.21-0.65). We controlled for variables of critical illness (shock index, IABP, anterior infarction, Qwave infarction) but could not find any influencing factor. We found significant longer decision times and times to hospital in the selfadmission group, but no difference in time to treatment or door to intervention-time. Patients who were admitted via EMS had higher CK levels that were reached after a longer inter-val (median 8 hours) and higher CK-Mb levels. We found a higher rate of typical symptoms and more patients with vegetative symptoms in the EMS group. There were more patients who had any revascularization in the EMS group. Conclusion: Patients with MI who were admitted at ED via private ways of transportation had significantly lower mortality. This could not be explained by factors of critical illness but rather by preferable use of EMS by patients with more severe symptoms and larger infarct size. Procalcitonin (PCT) -a predictor of outcome after CPR? Schusterschitz (PCT) is considered a parameter capable of distinguishing systemic inflammatory reactions caused by infections from those resulting from non-infectious states. However, the trigger for its secretion is still unclear and some investigators reported increased PCT also in non-infectious conditions like major abdominal surgery, heatstroke or treatment with T-cell antibodies. It was the aim of this study to test whether PCT is increased in the absence of infection after cardio-pulmonary resuscitation (CPR) and if so, whether it does correlate with survival and neurological outcome. Methods: 31 consecutive patients admitted to the ICU after CPR from March to November 2006. PCT, CRP, NSE and lactate were determined daily within the first 72 hours. ICU survival as well GOS (Glasgow outcome scale) at hospital discharge were obtained. All patients were intubated and received standard medical treatment. Results: 17 patients did not survive whereas 14 patients survived to hospital discharge. Of the surviving patients four were classified GOS 2, one GOS 3, two GOS 4 and seven GOS 5. PCT obtained within 24 hours of admission was significantly different between survivors (median: 0,31, IQR: 0.11-2.73) and non survivors (median: 5.64, IQR: 1.66-13.13, p < 0.01). A statistically significant difference between those two groups could also be found for NSE but not for CRP or lactate. In addition we found a highly significant correlation between GOS at hospital discharge and PCT-levels during the first 24 hours after ICU-admission (Pearson r = -0.56, p = 0.0018). Conclusion: PCT can be significantly elevated in non-infectious acute states like CPR. Moreover PCT appears to predict not only survival but also neurological outcome after CPR. year old woman was admitted to our hospital with slurred speech and a history of weakness of her left side 3 hours before. 2 days ago, she was seen at our out-patient clinic with right sided headaches, amaurosis fugax of her right eye and slight dizziness. She was diagnosed with migraine with aura and discharged home with painkillers. On admission she told us that her symptoms had appeared 2 hours after drinking one glass of sparkling wine with her friends but she usually doesn't really cope well when drinking alcohol. After 15 minutes the weakness of her left side disappeared completely. On examination, the patient was generally in good condition, afebrile and had no abnormal neurological findings, apart from slurred speech. Blood pressure was normal at 120/80 mm Hg, as was her visual acuity. The blood alcohol level was low at 12 · 3 mg/100 ml and CT scan of the brain was normal. In the evening of the same day she suddenly developed aphasia and a left-sided hemiparesis. We immediately repeated a CT scan and early signs of a stroke were detected (Fig. 1) . She underwent doppler sonography of her neck vessels and a dissection of the right carotid artery was seen (Fig. 2) . At that moment the patient suddenly developed generalised tonic-clonic seizures which had to be treated with diazepam. After an online telephone conference with the department of Neurology at the Medical University of Salzburg we decided to give her a double bolus of reteplase (an off label use), the only available thrombolytic medication in our stock. The patient's condition rapidly improved after the second bolus. She was then transferred there for further investigations and the dissection of the right carotid artery was confirmed with a conventional angiography (Fig. 3) . The spontaneous dissection of the carotid artery causes nearly 25% of strokes in patients under 50 years of age, with a distinct peak in the fifth decade of life [1] . Blood can enter either between the Tunica intima and Tunica media or Tunica adventitia of the vessel and can cause either a stenosis or a complete occlusion. This can occur without any symptoms, because of the brain's compensatory arterial circulation (circulus arteriosus Wilisii), although cerebral emboli can lead to ischemic stroke. Transient ischemic attacks or transient monocular blindness are warning signs for a spontaneous dissection of the carotid artery in patients prone to this. Only about 1/5 of patients have an ischemic stroke without warning signs [2] . The classic triad of symptoms (pain on one side of the neck, head or face, Horner's syndrome and later retinal or cerebral ischemia) is found in less than 1/3 of patients. 3/4 of people who have a history of migraine describe the pain as different from their usual migraine headache. 1/4 of patients mistake the pain as migraine [3, 4] . Amaurosis fugax is an important symptom and the reason for it can be severe. Patients should therefore be evaluated carefully. Attacks of migraine may start at any age but the incidence peaks in early to mid-adolescence. 15 percent of migraine sufferers may experience transient focal neurological symptoms, the majority being visual (migraine with aura). In patients over the age of 40, atherosclerosis should be excluded first because of the risk of cerebral and retinal stroke. Appropriate diagnostic examinations, such as Doppler sonography or angiography, are therefore indicated. In younger patients with amaurosis fugax of unknown cause, magnetic resonance angiography is indicated in preference to the more invasive angiography, which should be avoided if possible. Studies [5] [6] [7] have shown the safety and efficacy of intravenous thrombolysis in patients with acute stroke due to spontaneous carotid dissection. Duplex sonography is useful for screening patients who may have a dissection as an abnormal pattern of flow can be found in more than 90% of patients [8, 9] . To confirm the diagnosis an angiography or MR is indicated [10, 11]. Patients referred for acute CA suffering an acute STEMI or NSTEMI, their risk factors and in-hospital mortalities in these two consecutive winters were recorded. Results: As expected, average temperature was lower (-1.6 vs + 5.9 8C, p < 0.001) and humidity was higher (82 vs 79%, p < 0.012) in CW compared to WW with no significant differences in other weather conditions (rain/snowfall: 59 vs 39 days; sunshine: 3.9 ± 2.5 vs 4.3 ± 2.5 hours/day; air pressure: 713.0 ± 6.7 vs 713.8 ± 7.1 hPa). There were no differences in the number of overall CA (987 vs 983) in these two winters, whereas the number of acute CA (12.9% vs 10.4% of overall CA, p = 0.046) and the diagnosis of STEMI as indication for acute CA (74.0% vs 62.7%, p = 0.046) were higher in CW. Furthermore, patients in CW were younger (58.2 ± 12.4 vs 61.7 ± 11.7 years, p < 0.03), had higher LDL-cholesterol (134.8 ± 44.6 vs 116.7 ± 36.0 mg/dl, p < 0.003) and were less frequent hypertensives (52.8% vs 70.6%, p < 0.01). In-hospital mortality (2.4% vs 1.0%, p = NS), patients' nationalities (Austrians: 78.0% vs 77.5%), delays in STEMI treatment (3.9 ± 3.5 vs 3.8 ± 3.1 hours) and other traditional risk factors were not different between WW and CW. Conclusion: The dramatic average temperature increase of 7.5 8C from the cold to the warm winter was associated with a decrease in acute coronary angiographies and a lower incidence of STEMI referred for primary percutaneous intervention. However, in-hospital mortality was not different between the cold and the warm winter, probably due to the generally low mortality. Enteric glia (EGC) are the most numerous cell type in the enteric nervous system. It has been previously shown by our group that EGC participate in intestinal immune responses via their phagocytosis, induction of MHCII/ICAM1 by pro-inflammatory signals, conspicuous antigen-specific stimulation of syngeneic T-cells, and their production of proinflammatory cytokines. Here, we have assessed putative important pro-inflammatory signaling pathways in purified EGC cultures from rat intestine after exposure to bacterial peptidoglycans (PGN). To selectively expose EGC to PGN, we have used an H. pylori based model of intracellular translocation of PGNs. A strong peak of IjBa-degradation and nuclear translocation of p65 occurred 60-90 min after infection with the H. pylori wild-type (HPWT) strain proving NF-jB activation by immunoblot, whereas no significant IjBa-degradation could be detected after EGC infection with an isogenic mutant H. pylori (HPM) strain that is unable to convey the intracellular translocation of PGN. These results were confirmed by EMSA. To further analyze the signaling pathways induced by immune-activation of EGC, we studied activation of NOD in EGC upon PGN challenge. RICK (a downstream kinase of NOD leading to NF-jB activation) activity after intracellular PGN translocation was assessed by in vitro kinase assays. These assays demonstrated a dramatically upregulated RICK activity in EGC after PGN challenge with a maximum at 60 min after infection. In contrast, HPM-strain could not activate RICK in enteric glia. In vitro data were verified and reproduced in an in vivo infection mouse model using salmonella typhimurium. Surprisingly, immunohistochemical analysis revealed nuclear translocation of phospho-p65 in EGC in the colon 24h after infection, while the maximum of NF-jB activation in "commonly" immunoreactive cells like macrophages were detected after 48 h. In summary, evidence was provided that EGC strongly and early respond to intracellular PGN with activation of RICK/ NF-jB. These data support consideration of EGC as another celltype in gastrointestinal immunology. EGC may play a major role in the initiation process of intestinal bacterial translocation in sepsis by increasing the permeability of the intestinal wall through an induction of bacterial triggered inflammation. Kaneider N. C. 1 , an important regulator of endothelial barrier function and blood coagulation, has been proposed to be involved in the lethal sequelae of sepsis, but it is unknown whether activation of PAR1 is beneficial or harmful. Using a cell-penetrating peptide (pepducin) approach, we provide evidence that PAR1 switched from being a vascular-disruptive receptor to a vascular-protective receptor during the progression of sepsis in mice. Unexpectedly, we found that the protective effects of PAR1 required transactivation of PAR2 signaling pathways. Our results suggest therapeutics that selectively activated PAR1-PAR2 complexes may be beneficial in the treatment of sepsis. Consequently, during conditions of overwhelming production of oxidizing compounds it can be assumed that the intake of essential antioxidant vitamins can become insufficient for proper function of these enzymes, and thus hyperhomocysteinemia may develop even when vitamin supply is within normal. Because the clinical course of patients after multiple trauma and/or with sepsis is accompanied by strong inflammatory responses, we became interested to test whether in these patients hyperhomocysteinemia will develop even when they received standardized parenteral nutrition. In a prospective study, homocysteine concentrations were measured in plasma specimens from 18 patients (14 males, 4 females, Apache score: 18.9 ± 6.75, 8-34, ISS: 39 ± 13.1, 18-57) posttrauma or with sepsis during 12-14 days of follow up. All patients received standard parenteral nutrition (1700 kcal/d, 100 g amino acids/d) after the end of hypodynamic shock. Of every patient up to 5 specimens were included in this analysis, in total 84 specimens were available within the observation period. Compared to baseline, homocysteine (p < 0.05, d7 and d10) and cysteine concentrations increased during follow-up of patients (p < 0.05, d7, d10 and d13). During follow-up higher homocysteine concentrations were found in non-survivors than survivors (significant on d4 and d10). Actually, homocysteine levels only increased in patients who died (significant on d4, d7 and d10), in non-survivors homocysteine concentrations remained stable during follow-up. By contrast, cysteine concentrations increased in both groups of patients independent from their outcome (p < 0.05 on d7 and d10) and there was no significant difference between survivors and non-survivors. The predictive information of moderate hyperhomocysteinemia after trauma suggests its relationship to the inflammation status of patients. Because all patients received standardized parenteral nutrition, distinct B-vitamin supply is unlikely to explain the different development of homocysteine levels in survivors and non-survivors. Unlike homocysteine concentrations, cysteine levels increased but were not different between survivors and non-survivors. Its positive inotropic effect is mediated by sensitizing troponine C to calcium-ions, whereas the vasodilatory effect is due to activation of ATP-sensitive potassium channels. Moreover, this drug was successfully used in the treatment of patients with septic shock. We investigated the effects of levosimendan on the respiratory burst activity of polymorphonuclear leukocytes (PMNL) in vivo. PMNL were isolated from patients with severe cardiac failure or septic shock before and after levosimendan infusion. PMNL from healthy volunteers were incubated in levosimendan in vitro after isolation. Thereafter oxidative burst was stimulated with formyl-MetLeuPhe (fMLP) or phorbol myristate acetate (PMA) and was quantified using DCF-DA. In vivo treatment of PMNL with levosimendan resulted in reduced oxidative burst activity in unstimulated as well as in fMLP-or PMAstimulated cells. The most pronounced effect was seen in the reduction of basal oxygen radical production. In vitro treatment of isolated neutrophils from healthy volunteers with the drug showed a biphasic dose-response relationship. Low doses of levosimendan induced and augmented respiratory burst activity in PMNL, whereas a higher dosis inhibited the respiratory burst comparable to the in vivo experiments. In addition to the well known inodilatory activity, levosimendan might have immunomodulatory effects by influencing polymorphonuclear neutrophils reactive oxygen species production. Inflammatory response of uromodulin-expressing renal proximal tubular cells play an active role in the recruitment of inflammatory cells into the renal interstitium, further mediating initiation and progression of renal cell damage. Uromodulin, the most expressed urinary protein, is involved in the immunomodulatory response (i.e. activation of neutrophils and immunoglobulin-binding) and is a pivotal factor in the pathogenesis of some hereditary renal diseases. We investigated the influence of uromodulin-expression in porcine proximal tubular cells (LLC-PK1) on transepithelial migration (TEM) of polymorphonuclear cells (PMN) across LLC-PK1 monolayer. PMN transmigration measurements were performed in control (spontaneous TEM), after stimulation of LLC-PK1 with different concentrations of TNF-a or LPS, and after preincubation with IgG. Uromodulin-expressing (Umod(+)) cells allow higher spontaneous TEM rates than non-uromodulin-expressing (Umod(-)) cells. TEM across Umod(+) cells could not be further enhanced by pre-incubation with TNF-a, which instead stimulated TEM across Umod(-) cells. Furthermore pre-treatment with IgG aroused TEM through Umod(+) cells. In conclusion, our results show new insights into the inflammatory potential of renal tubular cells and in particular, the possible involvement of uromodulin in enhancing the inflammatory processes by leukocyte activation. To evaluate the impact of including antithrombin levels (AT) in the score. Patients/Methods: A retrospective observational study conducted in 364 patients in critical care. Coagulation parameters obtained daily for DIC screening were utilized for scoring. Results: There were 194 patients identified with nonovert DIC scores including utilization of AT levels. As compared with patients without non-overt DIC, these non-overt DIC patients had significantly higher mortality (15.5% vs 4.1%, p < 0.0001). Of these, 34 patients developed overt-DIC ISTH after admission; positive non-overt DIC scoring with AT proceeded positive overt-DIC scoring by a median of 1.3 days, which was significantly earlier than a median of 0.1 days in non-overt DIC scoring without inserting AT levels (p = 0.004). 30 patients developed overt-DIC JMHW after admission, in whom the positive non-overt DIC scoring with AT proceeded positive overt-DIC JMHW scoring by a median of 2.6 days, which was significantly earlier than a median of 2.0 days with non-overt DIC score without AT (p = 0.04). Conclusions: Non-overt DIC scoring as proposed by the ISTH predicted a high risk of death in critically ill patients. Die Kriterien zur Aufnahme von Zirrhosepatienten an eine Intensivstation sind jedoch sehr uneinheitlich, sodass wir in einer retrospektiven Analyse die ICU-Mortalität, Krankenhausmortalität und 1-Jahresmortalität von Patienten, die mit einer Zirrhose an unserer Intensivstation aufgenommen wurden, ermittelten. Patienten: 131 Patienten mit Zirrhose (118 männlich, 13 weiblich) und einem mittleren Alter von Gesamt-ICU-Mortalität 11,3%), die Krankenhausmortalität 34,35% (gesamt 18,2%), die kumulative 1-Jahres-Mortalität 51,9%. Der MELD Score (30,64 vs. 15,97), der APACHE II Score (13,08 vs. 24,47) und der SAPS II Score (27,53 vs. 52,71), sowie die mittlere Dauer der maschinellen Beatmung in Tagen (0,77 vs. 3,62) unterschieden sich signifikant (p < 0,01 -Mann-Whitney U-Test) zwischen überlebenden und im Krankenhaus verstorbenen Patienten. Patienten mit oberer gastrointestinaler Blutung wiesen eine Krankenhausmortalität von 15%, mit hepatischer Enzephalopathie von 22% und mit hepatorenalem Syndrom von 64,3% auf. Schlussfolgerung: Intensivpatienten mit Leberzirrhose haben mit 34% eine deutlich höhere Krankenhausmortalität als der Durchschnitt der Intensivpatienten. Ein Jahr überlebten weniger als 50%. Alle Patienten mit einem APA-CHE II Score unter 11 bzw. einem SAPS II Score unter 21 überlebten den Krankenhausaufenthalt, alle mit einem MELD Score über 42 verstarben noch im Krankenhaus. Über seine Gewebegängigkeit beim Menschen ist bislang wenig bekannt. In der vorliegenden Studie wurden Gewebekonzentrationen in Autopsieproben gemessen. Gewebeproben aus der Lunge, den Nieren, der Leber, der Milz, dem Myokard und aus verschiedenen Gehirnregionen wurden im Rahmen von Routineautopsien zweier Patienten, die unter VRC-Therapie verstorben waren, entnommen. VRC war aufgrund vermuteter pulmonaler Aspergillose verabreicht worden. Patient 1 hatte einmalig eine Dosis von 200 mg VRC, Patient 2 eine Gesamtdosis 2000 mg erhalten. Beide Patienten waren unter Vasopressor-und Respirator-Therapie im Multiorganversagen verstorben. VRC wurde aus homogenisiertem Gewebe extrahiert und mittels Hochleitungsflüssigkeitschromatographie quantifiziert. In den Nieren wurden die höchsten Konzentrationen gemessen (Patient 2: 6,89 ± 0,06 lg/ml [Mittelwert ± Standardabweichung]), in der Leber die zweithöchsten (Patient 2: 4,21 ± 0,77 lg/ml). Im Lungengewebe betrugen die Spiegel 0,74 ± 0,02 lg/ml (Patient 1) und 1,87 ± 0,27 lg/ml (Patient 2). In der Milz war die Konzentration 1,31 ± 0,03 lg/ml in den Proben von Patient 1 und 2,95 ± 0,05 lg/ml in den Proben von Patient 2. In den Hirngewebeproben aus Cortex, Hippocampus, Nucleus caudatus, Medulla oblongata und Cerebellum von Patient 1 lagen die VRC-Spiegel unterhalb der Nachweisgrenze ( < 0,25 lg/ ml). Im Gehirn von Patient 2 erreichte die VRC-Gewebekonzentration 3,34 ± 0,18 lg/ml. Es ergaben sich keine wesentlichen Unterschiede zwischen den verschiedenen Hirnregionen. Auch im Myokard von Patient 1 konnte kein VRC nachgewiesen werden; in den entsprechenden Proben von Patient 2 wurde eine VRC-Konzentration von 2,44 ± 0,25 lg/ml gemessen. VRC erreichte bereits nach der ersten Dosis in den meisten Geweben nachweisbare Konzentrationen, wobei es sich offenbar in der Niere und in der Leber anreichert. Im Gehirn und im Myokard werden niedrigere VRC-Spiegel erzielt und die Penetration in diese Gewebe dürfte langsamer erfolgen. The vascular endothelium represents a complex regulated surface, which maintains an antithrombotic potential. This pattern is shifted towards a prothrombotic state, when endothelial cells shed fragments of their plasma membrane into the extracellular space in response to injurious stimuli. Such fragments, known as microparticles (EMPs), are elevated in a variety of diseases associated with endothelial dysfunction. This investigation aims to further characterize EMPs in patients with septic shock and to examine the influence of EMPs on coagulation. The study comprised 15 critically-ill patients with septic shock and 18 healthy volunteers. EMPs in platelet-poor plasma (PPP) were measured by flow cytometry with fluorescent antibodies (anti-CD144, anti-CD62E) allowing discrimination of EMPs from platelet microparticles. In a subset of 5 patients EMPs were freshly isolated from PPP and procoagulant activity of EMPs was quantified by determining clotting times with a coagulometer. Furthermore EMP levels of tissue factor (TF), the initiator of coagulation, were determined by ELISA. Plasma from patients with septic shock contained significantly higher levels of CD144 and CD62E double-positive EMPs as compared to healthy volunteers. Clotting time analyses revealed a markedly increased procoagulant activity of EMPs of patients with septic shock. Additionally, the ELISA quantification of TF in freshly isolated EMPs showed the presence of high levels of TF, well above the values reported for healthy patients. In conclusion, patients with septic shock have high levels of CD144-and CD62E-positive EMPs which exhibit pronounced procoagulant activity and high levels of TF. Thus, EMPs may contribute to activation of coagulation cascade in severe sepsis by increasing the levels of circulating TF. [1] . Methodik: Meerschweinchen wurden getötet und der Dünndarm entnommen. Nach einer einstündigen Ruhephase wurden ca. 8 cm lange Dünndarmsegmente in parallel angeordneten Organbädern eingespannt und mit Tyrodelösung (einer Nährlösung) endoluminal durchströmt. Nach einer neuerlichen Ruhephase wurde der Abfluss der Tyrodelösung erschwert, und dadurch ein peristaltischer Reflex ausgelöst. Durch Messen des pressure threshold (PT), des Triggerpunktes ab dem ein peristaltischer Reflex getriggert wird, kann der Einfluss verschiedener Substanzen auf die Peristaltik evaluiert werden. So wurden Tauroursocholat (TUDCA, 0,025-1,6 mM) und Taurocholat (0,025-0,075 mM) in ansteigenden Konzentrationen intraluminal verabreicht und der PT gemessen. Im zweiten Schritt wurde der Effekt der Lösungsmittel (Tyrode und DMSO) auf die Peristaltik evaluiert. Eine Stimulation der Peristaltik zeigt sich in einer Senkung, eine Hemmung in einem Anstieg des PT. Ergebnisse: Taurocholat führte zu einem diskreten, nicht statistisch signifikanten Anstieg des PT. Im Gegensatz dazu zeigte sich unter TUDCA eine statistisch signifikante (p < 0,01), konzentrationsabhängige Hemmung des peristaltischen Reflexes. Bei der höchsten getesteten TUDCA-Konzentration (1,6 mM) kam die Peristaltik völlig zum Erliegen. Weder DMSO noch Tyrode hatten einen nachweisbaren Effekt auf die Peristaltik. Schlussfolgerung: Nur eine der getesteten Gallensäuren konnte die Peristaltik beeinflussen, der zugrundeliegende Mechanismus ist noch unklar; eine mögliche Hypothese wäre die Wirkung der Gallensäuren über Opioidrezeptoren vergleichbar mit der Pathogenese des Pruritus bei Patienten mit einem Ikterus [3] . Effective treatment of refractory hypercalcemia due to primary hyperparathyreoidism by CVVHD using regional citrate anticoagulation -a case report Djanani A., Bellmann R., Dunzendorfer S., Joannidis M. Department of Internal Medicine, ICU, Medical University Innsbruck, Austria A 53 year old lady with a history of depression presented to the psychiatric outpatient unit of a tertiary hospital with worsening apathy and significant weight loss (6 kg) within the last 3 months. Her major complaints included bone and abdominal pain. Initial investigations revealed drastically elevated total serum calcium (6.08 mmol/ l) and the patient was immediately referred to the medical ward. During the following 48 hours serum calcium increased despite volume expansion and treatment with diuretics, calcitonin, zoledronic acid and the patient developed an acute kidney injury. Because of the development of severe sepsis associated with pancreatitis and deteriorating mental state the patient had to be intubated and transferred to the medical intensive care unit of the university hospital of Innsbruck. The CT scan showed an adenoma of 3.3 × 3.5 × 6.1 cm next to right lobe of the thyroid gland, an enlarged liver and bilateral pulmonary infiltrates. Together with highly elevated PTH (2217 ng/l) these results confirmed the diagnosis of primary hyperparathyreoidism. At that time the patient was anuric. To enable resection of the adenoma under safe conditions normalization of serum calcium was required before surgery. First intermittent hemodialysis was initiated which reduced serum calcium during each treatment session but a rebound occurred within a few hours. Consequently, we switched the patient to CVVHD using citrate anticoagulation with the intention to normalise serum calcium by constant chelation and elimination of calcium. Regional citrate anticoagulation was performed by infusing 4.0% tri-sodium citrate into the extracorporeal circuit using a calcium free dialysate solution with reduced sodium (133 mmol/l) and bicarbonate content (20 mmol/l) applying a dialysate flow of 2000 ml/h. Calcium substitution was reduced to fifty percent of the usually applied dose. With this method we were able to normalize elevated serum calcium within 48 hours. Surgery had to be deferred until pneumonia and sepsis resolved. On day 14 after admission to the ICU the adenoma could be successfully removed. One month later the patient could be transferred back to the referring hospital still requiring intermittent hemodialysis but already producing spontaneous urine. This case demonstrates the ability of CVVHD with regional citrate anticoagulation to easily control severe hypercalcaemia.
