Within 24 hours of institution of therapy with penicillin, it is difficult to isolate group A streptococci from patients with acute streptococcal pharyngitis, and infected children can return to school. In general, patients with a negative result of the rapid test do not require treatment before culture verification unless there is a particularly high suspicion group A streptococcal infection (e.g., scarlet fever, peritonsillar abscess, or tonsillar exudates in addition to tender cervical adenopathy, palatal petechiae, fever, and recent exposure to a person with group A streptococcal pharyngitis).
