The health care system in which these fatal and non-fatal errors occur covers a spectrum that stretches from the manufacturers of equipment and drugs to the cleaning crew in the operating room and it involves many different clinical and supportive departments and their personnel.Anesthesia is an important component of this health care system comprising many interdependent parts that can affect the quality of anesthesia care.
