COST Action 15222 'Cost Cares' was funded by the EU Commission to create the impetus in both the research and development required to design and test innovative exploratory health laboratories (EHLs) to implement PCC and HP across the EU. Repeated here for clarity the critical enablers are (1) information technology (IT), which describes the use of computers or other computerized devises to store, transmit, and receive data to support PCC planning and care coordination, for handling and communicating health and evaluation data, and for delivering PCC and HP interventions.
