Focus groups were conducted in acute and community/primary care settings from large urban (Edmonton, Calgary), moderate-sized regional (Red Deer), and rural (Slave Lake) centers in Alberta (Canada) between October 2015 and February 2016 to seek input from both patients with COPD and health care providers. The analysis identified four emerging themes influencing COPD discharge care bundle implementation as discussed by participants: (1) process of care, (2) human capacity in care setting, (3) communication and engagement, and (4) attitudes and perceptions towards change.	./cache/cord-031558-8wysernx.txt	./txt/cord-031558-8wysernx.txt
